To optimize workflow, it is a normal to insert a risk, qualify severity, likelyhood, ... and entering mitigation activites. Same for issues.
Request is: In box "linked actions", there should be a selection "New activity". This should start form for entering a new activity, link them automatically to risk (or isse) and on save to return to risk (or issue) where you started from.
