Since severals updates, Risk module is not completely operational. Now, when you enter a Risk, the system ask you to enter an action and, Risk is closed automatically. It is not possible to enter a Risk and track it (affected, worked, closed,...... worklow)
Is it a new rule implemented or a bug ?

We dont understand your problem, this feature is not implemented on projeqtor, your admin maybe maked some configuration, ask him how its configurated.
if is not a admin configuration take screenshot of your problem and post this here.

Yes, Risks management is an old standard feature, but "he system ask you to enter an action and, Risk is closed automatically" is not.
You can test on demo, what you describe is not the default behavior.
I guess your admin added some customization :
- to set result mandatory when status is "done"
- checked status "in progress" to set "closed" meta-status
- set "inprogress' as first status in the workflow.
