If possible, I'd like to give a +1 to ticket "1114 - Document approvment improvement", so it could be delivered before (maybe in the next version? 😉 ), since it is planned for version 5.
I know there are a lot of document management improvements recorded, but I believe approving or reproving a document is something essential.
My suggestion to keep it simple: add one more button (reprove). When the user click it, open a dialog so the user can type the reason for reproving the document. (to keep things simple, this reason could be added to notes, so it wouldn't required a specific place for it). The user then should press a OK button, otherwise the document would not be considered reproved, but still pending.
