I followed the steps you suggested, but when I checked status, it was found duplication for status: submitted, rejected, accepted !
I don't know how it's duplicated!
I think you may help me to find that when and how it happened.
I doubt, it's happening through adding new workflow.

You get duplicated list of statuses.
Try and delete duplicates.

Yes, I will remove duplicate statuses, I was interested to know, how it happened?!

... duplicate first install
... export / import of statuses
... play with the DB
Only you (and mayby God, but I'm not sure) can know what went wrond are there is no spontaneous generation in the DB.

But I am sure that I have not done anything what you have mentioned.
I mentioned date from history showing in status & it was only performed an action of adding workflow on that date.

I am just trying to know the reason, but anyway if i find how it was happened, I will surely share with you.
