I have tested this now with ACTIONS, ISSUES, RISKS, OPPORTUNTIES and creation of an email template. In all cases the table was created for HIstory. But no entries were shown. I have attached a document with screen shots of issue.

The $HISTORY is done to display "Last Changes" (it is clear when you use helper to edit template and on the table header)
The idea was to be able to display what was changed by the event which sent the mail (for automatic mail)
So it displays only the changes done during the last 10 second from mail sending.
Possibly it could be an idea to split and have $HISTORYFULL for whole history
I'll write this in the roadmap
